Position Highlights
Maybe eligible for $10,000 sign on bonus!
Monday-Friday 8am-5pm with occasional night, weekend, and holiday. On Call involvement.

A Brief Overview
Provides major portions of self-care and home dialysis training and monitoring for dialysis patients and/or their caregivers.
What you will do

Assesses patient pre-therapy to determine if patient meets selection criteria.

Identifies patient/family learning needs; Develops, implements and evaluates an educational plan for patients; Provides patient education related to but not limited to the dialysis process and procedures related the that process, including care and management of dialysis accesses.

Monitors and evaluates patients, on a minimum of a monthly visit, to problem solve/mentor patient and families to safely deliver home therapy.

Problem-solves and facilitates appropriate resources for patients on home therapy.

Coordinates care of patients in collaboration with Medical Director/Nephrologist, patients, families, other nursing staff and ancillary departments.

Maintains accurate documentation and computer skills.

Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental and infection control standards.

Documents in the patient record according to departmental policy/procedure.

Promotes and maintains documentation and an environment that is in compliance with state, federal and regulatory agencies, including CMS/ESRD-Renal Network, and Quality Assurance and Process Improvement efforts.

Facilitates patient supply re-order.

Certified Hemodialysis Nurse (CHN) - Board of Nephrology Examiners Nursing and Technology (BONENT) Upon Hire or

Certified Peritoneal Dialysis Nurse (CPDN) - Board of Nephrology Examiners Nursing and Technology (BONENT) Upon Hire or
-
Certified Dialysis Nurse (CDN) - Nephrology Nursing Certification Commission (NNCC) Upon Hire or
-
Specialty Certification - The American Society of Histocompatibility & Immunogenetics (ASHI) Upon Hire
-
Dialysis experience or critical care experience
